TikTok’s Alternative Soda Trend Could Be Tough on Teeth

New study sheds light on acidic, sugar-free beverages

New research finds acids in sugar-free beverages could erode tooth enamel, as a recipe mixing flavored sparkling water with balsamic vinegar to create a so-called “healthier” alternative to soda takes TikTok by storm.

The research, published online in the open access journal JADA Foundational Science, adds to our understanding about the importance of limiting consumption of acidic beverages. The TikTok video of adding balsamic vinegar (an acid) to flavored sparkling water (an acidic beverage) recently received more than 6.3 million views.

“I love balsamic vinegar, but I enjoy it more on my salad than in my drinking glass. It’s much kinder to the teeth than bathing them in a beverage blend of two acids,” says Dr. Edmond Hewlett, a spokesperson for the American Dental Association. “The more acidic the drink, the greater the risk of tooth erosion with frequent consumption.”

Acidic foods and beverages can wear away the enamel that protects teeth, a process known as tooth erosion. Tooth erosion is permanent and may open the door for bacteria to cause cavities or infection. It can also make teeth appear discolored, as white enamel is worn away to expose the yellow layer below called dentin.

Research and results

In the study, researchers wanted to see whether non-carbonated bottled water, flavored sparkling water and plain sparkling water might cause dental erosion. Recently extracted human teeth were soaked in 7 different sugar-free beverages (and one soda with sugar for comparison) to see which, if any, beverages caused erosion. Teeth were exposed for twenty-four hours, which the researchers considered to replicate a year’s worth of exposure to these beverages.

When measuring the results of soda with sugar versus sugar-free soda, they found acids in both caused dental enamel to erode. Sweetener type was less of a factor, as it was the acid in the beverage that eroded the enamel.

Researchers also observed erosion in flavored sparkling waters, though it was less than that observed for sugar containing and sugar-free soda. The only beverages in the study that did not erode enamel were non-carbonated, non-flavored bottled waters.

Consuming acidic foods and beverages

While the TikTok recipe was not included in the study, it is similar to the other sugar-free, acidic beverages tested.

“People find carbonated beverages refreshing, especially this time of year. Enjoy them in moderation and preferably with meals,” Dr. Hewlett says. “But if you’re looking for a glass of something that is actually good for your dental health, regular water, including fluoridated tap water, or milk are always good options.”

The American Dental Association offers these tips to reduce tooth erosion from acidic food and beverages:

● Use a straw, sip and swallow – do not swish or hold it in your mouth longer than you need to.

● Wait an hour before brushing to give saliva a chance to naturally wash away acids and re-harden enamel.

● Rinse with water, drink milk or enjoy a snack of cheese right afterward. Dairy and other calcium-rich foods can help neutralize acids.

● Keep saliva flowing to keep acids under control and protect your teeth. You can do so by chewing sugarless gum.

● Look for dental health products like enamel erosion control toothpaste and sugarless gum with the ADA Seal of Acceptance, which indicates the product has been rigorously and independently evaluated by dental experts.

● Practice good oral health every day: Brush twice a day with fluoride toothpaste, clean between your teeth daily with floss or another interdental cleaner, eat a healthy diet that limits sugary foods and beverages, and visit a dentist regularly.

New Research Confirms the Majority Of Americans Mercury Exposure From Dental Amalgam Mercury Fillings Exceeds California Safety Limit

The International Academy of Oral Medicine and Toxicology (IAOMT) is raising awareness of research linking highly significant elemental mercury excretion to the presence of dental amalgam fillings in the mouth. These so-called "silver" fillings also called amalgams are actually 50% or more mercury and are widely used in the United States, in all branches of the military, low-cost insurance and disadvantaged children and adults.

In the present study, researchers David and Mark Geier reviewed the urinary mercury excretion of over 150 million Americans using the CDC's 2015-2018 National Health and Nutrition Examination survey (NHANES). The Geiers found a highly significant relationship between the number of dental amalgam filling surfaces in the mouth and amounts of mercury excreted. They compared the amounts of mercury being excreted to the current mercury minimum risk levels of both the US EPA and California's EPA.

It is important to note that the number of surfaces is not the same as the number of fillings. Each tooth has five surfaces, which means that a person with only one filling could have up to five surfaces.

Of the 91 million (57.8%) adults who had 1 or more surfaces of mercury fillings, the amount of mercury in their urine correlated significantly with the number of surfaces of amalgam. The Geiers wrote that, "Daily Hg vapor doses from amalgams were in excess of the most protective California's Environmental Protection Agency (EPA) safety limit for about 86 million (54.3%) adults". The US EPA minimum risk level (MRL) for mercury is considerably higher than CalEPA's MRL due to the fact that by statute CalEPA's MRL must protect the vulnerable, not the average. However, 16 million adults are exposed to levels of mercury greater than the US EPA's MRL.

Similar information about excessive exposure was presented by the IAOMT at the FDA expert hearing on the safety of amalgam in 2010 and one dentist on the panel asked the experts from the Agency for Toxic Substances and Disease Registry (ATSDR) how much over the MRL can you go and still be safe. Dr. Richard Kennedy of ATSDR explained that one cannot exceed the MRL and still expect to be safe.

In September 2020, the Food and Drug Administration (FDA) updated risks of dental amalgam fillings for susceptible groups and identified fetal exposure during pregnancy as the most critical exposure and recommends no amalgam fillings for women from fetus to menopause because of that risk. Additionally, the FDA recommended that children, people with neurological disease such as multiple sclerosis, Alzheimer's disease or Parkinson's disease, people with impaired kidney function, and people with known heightened sensitivity (allergy) to mercury or other components of dental amalgam avoid having these mercury fillings placed.

"Toxic mercury vapors are continually off-gassed from dental amalgam fillings with stimulation such as chewing," explains David Kennedy, DDS, Past IAOMT President. "With the Geiers' new research joining the ranks of hundreds of other studies, it is abundantly clear that mercury from amalgams pose a danger to everyone, including unborn babies, patients, dentists, and dental employees."

The Geiers' study was partially funded by the IAOMT, a non-profit organization that evaluates biocompatibility of dental products, including mercury filling risks.

At the ceremony, HU President Asher Cohen paid tribute to Mr. Bergman, "in acknowledgment of his strong commitment to the State of Israel, the Jewish community, educational initiatives, and cultural diversity; in admiration of his professional achievement as the CEO and Chairman of the Board of Henry Schein Inc.; in recognition of his contributions to the Hebrew University of Jerusalem, particularly within the Faculty of Dental Medicine and his Bridge to Peace initiative to provide prevention and treatment to patients with special needs."

Recognizing the accolade, Mr. Bergman said, “Our decades-long relationship with the Hebrew University of Jerusalem and the School of Dental Medicine is premised on our shared goals of expanding access to oral care for all underserved communities and enhancing educational opportunities in dentistry. On behalf of Team Schein, we are particularly pleased to accept this recognition in honor of our partnership to support the important work to improve health outcomes for the diverse populations throughout the Middle East and in the special needs community.”

For decades, Mr. Bergman has been a leader in promoting the role of business in making a positive difference in the world. He is actively involved with The World Economic Forum, where he has served as co-chair of the Health and Healthcare Governors Community. As co-founder of the Higher Ambition Leadership Alliance, Mr. Bergman has further sharpened Henry Schein’s commitment to corporate citizenship while also sharing best practices with like-minded companies. Mr. Bergman is an honorary member of the American Dental Association and the Alpha Omega International Dental Society and received an honorary fellowship from King’s College London – Dental Institute and the International College of Dentists. He has also received honorary doctorates from The University of the Witwatersrand, Western University of Health Sciences, Hofstra University, A.T. Still University's Arizona School of Dentistry and Oral Health, Case Western Reserve University, and Farmingdale State College (SUNY).

Mr. Bergman is co-founder of the India-Israel Forum and on the board of HU, as well as many others. Building on their long-term relationship, Henry Schein and the Hebrew University’s Dental Faculty partnered, along with others, to create the Bridge to Peace initiative in 2007 to provide preventive oral health care and treatment to patients in the Middle East. In 2017, the American Friends of the Hebrew University awarded Mr. Bergman and his wife, Dr. Marion J. Bergman, with the national Scopus Award to acknowledge their outstanding commitment and contribution to the Hebrew University, the State of Israel, the Jewish people, and humanity. Mr. Bergman also has been actively involved with the American Jewish Committee (AJC), serving as President from 2013 to 2016 and co-founder with his wife, of the AJC’s Africa Institute. Mr. Bergman is currently co-chair of the Muslim-Jewish Advisory Council.

The Bergman family has also been active supporters of organizations fostering the arts, higher education, human rights, cultural diversity, grassroots health care, and sustainable entrepreneurial economic development initiatives in the United States, Africa, and other developing regions of the world.

Mr. Bergman is a graduate of the University of the Witwatersrand in South Africa, and is a South African Chartered Accountant and a New York State Certified Public Accountant (CPA).
